Oklahoma Skin Care by Kevin McMullen MD

Don’t want any incisions when contouring pain-free? Contoura is a non-invasive procedure that doesn’t require any incisions and delivers noticeable results. Our non-invasive body treatments help improve the appearance of cellulite, reduce dimpled skin, help slow the loss of elasticity on the abdomen, love handles, hips, thighs and other areas that require fat reduction. A.C.E. technology targets deep within the skin to ensure that no areas are under, or over-treated, thereby maximizing results and providing consistent outcomes.

  • Plus is a non-invasive sub-dermal heating applicator that stimulates sub-cutaneous tissue. Radio-frequency energy flows uniformly between the electrodes to provide a comfortable thermal experience, built-in temperature controls allow for the attainment of clinically proven optimal temperatures without concerns of hot spots or under treatment.
  • BodyFX addresses problematic fatty tissue in larger body areas such as the abdomenback (flanks), and thighs. BodyFX works by using different clinically proven modalities including radio-frequency energy and negative pressure applied to the skin and underlying fat.
  • MiniFX is non-invasive contouring and cellulite technology that delivers a combination of clinically proven modalities, similar to the BodyFX, however, addresses problematic fatty tissue in smaller areas like the neckarms, and knees.

Submental Fullness

Do you have some extra fullness beneath your chin? Maybe it’s something you’ve had your whole life, something that developed over the years, or something you noticed recently. This is a condition called submental fullness, which some people refer to as “double chin.”

Both men and women may be affected by submental fullness,
which can be caused by:

You don’t have to be overweight to have fullness under your chin. And sometimes, no matter how much you exercise or diet, the condition may not go away.

You’re not alone. According to a 2015 survey conducted by the American Society for Dermatologic Surgery (ASDS):

67 % of people surveyed said they are bothered by the fat beneath their chin*

In fact, the survey results showed that people are just as bothered by submental fullness as they are by lines and wrinkles around and under their eyes.*

If you’re interested in a nonsurgical way to reduce fullness beneath your chin, KYBELLA® may be right for you. KYBELLA® is a first-of-its-kind injectable treatment for adults with submental fullness that destroys fat cells under the chin to improve your profile.

American Society for Dermatologic Surgery 2015 Consumer Survey on Cosmetic Dermatologic Procedures (N=7,315); Exact survey language was, “How bothered are you by excess fat under the chin/neck?” 69% of people responded they were bothered to the question: “How bothered are you by lines/wrinkles around/under the eyes?”

How Kybella Works

The active ingredient in KYBELLA® is synthetic deoxycholic acid. Deoxycholic acid is a naturally occurring molecule in the body that aids in the breakdown and absorption of dietary fat. When injected into the fat beneath the chin, KYBELLA® destroys fat cells, resulting in a noticeable reduction in fullness under the chin. Once destroyed, these cells can no longer store or accumulate fat, so further treatment is not expected once you reach your desired aesthetic goal.

Submental fullness is a gathering of excess fat beneath the chin, commonly known as “double chin.” It can be something you’ve had your whole life due to genetics, or start to appear as you get older or if you gain weight.

KYBELLA® is a prescription medicine and is the only FDA-approved injectable treatment used in adults to improve the appearance and profile of moderate to severe fat below the chin (submental fat), also called “double chin.”

It is not known if KYBELLA® is safe and effective for use outside of the submental area or in children less than 18 years of age.

This results in a noticeable reduction in fullness under the chin, revealing an improved chin profile.

Dr. McMullen will create a tailored treatment plan based on your chin profile. At each treatment, you will be given multiple small injections under your chin. You may receive up to 6 treatment sessions, spaced at least 1 month apart. Many patients experience visible results in 2 to 4 KYBELLA® treatments.

59% of patients received 6 treatments in clinical studies.

In clinical studies, results for KYBELLA®-treated patients who had

A ≥1-grade composite combined physician- and patient-rated improvement were 8% (1 treatment), 28% (2 treatments), 43% (3 treatments), 55% (4 treatments), 66% (5 treatments) and 72% (6 treatments), compared with 2% (1 treatment), 7% (2 treatments), 12% (3 treatments), 14% (4 treatments), 15% (5 treatments), and 22% (6 treatments) of patients treated with placebo

A ≥2-grade composite combined physician- and patient-rated improvement were 0% (1 treatment), 0.2% (2 treatments), 0.8% (3 treatments), 3.5% (4 treatments), 3.9% (5 treatments) and 15% (6 treatments) compared with 0% (1 treatment), 0.2% (2 treatments), 0.2% (3 treatments), 0.5% (4 treatments), 0.2% (5 treatments), and 1.5% (6 treatments) of patients treated with placebo

KYBELLA® can cause serious side effects, including trouble swallowing and nerve injury in the jaw that can temporarily cause an uneven smile or facial muscle weakness. In clinical studies, nerve injury in the jaw resolved on its own in a median of 44 days (range of 1 to 298 days), and trouble swallowing resolved on its own in a median of 3 days (range of 1 to 81 days).

The most common side effects are swelling, bruising, pain, numbness, redness and areas of hardness around the treatment area. These are not all of the possible side effects of KYBELLA®. Call With Dr. McMullen for medical advice about side effects.

When injected into fat under the chin, KYBELLA® causes the destruction of fat cells. Once destroyed, those cells cannot store or accumulate fat. After reaching your desired aesthetic, further treatment is not expected.

KYBELLA® has been the focus of a global clinical development program involving over 20 clinical studies. More than 1600 people were treated with KYBELLA® in these studies.

Only you and Dr. McMullen can decide if KYBELLA® is right for you. If you’re bothered by fullness under your chin and wish to avoid surgery, ask your doctor about KYBELLA®.

It is not known if KYBELLA® is safe and effective for use outside of the submental area or in children less than 18 years of age.

image_1889825_right About Pellevé

The Pellevé System delivers constant, gradual radiofrequency energy deeply into the skin, causing heat to build up where the skin and fat layer meet. The increasing heat modifies the collagen bundles deep in the skin, causing contraction and stimulating the growth of new collagen over time. The result: tighter, via soft tissue coagulation, younger-looking skin.

You can feel the Pellevé difference. The Pellevé treatment is virtually painless and generally quite pleasurable, unlike many other energy-based procedures. It is a very comfortable skin tightening treatment that takes less than an hour. You can go back to work and resume your normal routine right away, and no one will know you have had anything done. You will appear more radiant!

The wrinkle reduction effects of the Pellevé treatment may continue to improve as your skin produces new collagen. In a clinical study, after one treatment, 9 out of 10 patients experienced a significant benefit. At 6 months, almost 90% of those patients still had a lasting effect.

Pellevé FAQs

Safety of Pelleve

Yes. The Pellevé System received FDA clearance in 2009 for the treatment of mild to moderate facial wrinkles.

Yes. The Pellevé System was granted a CE Mark in 2005.

Clinical studies have proven that the Pellevé treatment is a safe and effective way to reduce wrinkles without surgery. The side effects and discomfort are minimal, and no drugs or anesthetics are used during the procedure.

A physician or qualified medical personnel under the direction of a physician may perform the Pellevé procedure.

During Pellevé

Each treatment will be from 30 – 60 minutes depending on the number of areas being treated at one time.

There is generally no preparation required before having a Pellevé treatment but please arrive well hydrated. Ask your doctor if you should discontinue certain topical products or medications.

First, a conductive gel will be applied to the areas to be treated. Patients say they feel a deep warming sensation when the Pellevé handpiece is applied to your skin. You will act as a guide to describe the depth of the sensation as your clinician moves the handpiece gently across the treated areas.

No. Unlike other skin tightening systems, a Pellevé treatment does not require anesthesia. A Pellevé treatment is virtually painless and in fact, patients comment that it feels like a warm massage.

After Pellevé

There is no recovery time needed after a Pellevé treatment. You may be slightly pink with mild swelling for a few hours, you can wear makeup right away and resume normal activities, so it fits easily into busy schedules.

Most doctors will instruct you to wear a broad-spectrum sunscreen with an SPF 30 to prevent future photo damage and maintain the results of your Pellevé treatment.

In most cases, patients see a temporary improvement in their skin for the first 24-48 hours following treatment. However, the process of regenerating collagen takes time, and therefore full results will continue to improve over the next several months.

87.1% of patients in the FDA clinical trial showed continued improvement at six months following treatment.

Many people see results immediately after just one treatment. For many patients, a series of three treatments spaced about one month apart may be recommended for optimal improvement. Additional treatments may be undertaken at any time for long term maintenance.

Many physicians combine Pellevé treatments with other cosmetic treatments such as wrinkle relaxing agents and dermal fillers. Only your doctor can recommend the best treatment plan for you, based on your skin type, age, and your cosmetic goals.

Chemical peels are a perfect way to pamper yourself and great monthly maintenance for your skin because they are quick, painless and effective.

Permanent makeup, also known as permanent cosmetics, Intradermal Pigmentation, or Micropigmentation, is a method of applying natural pigment into the dermal layer of skin. It is most commonly used in creating permanent eyeliner, permanent eyebrows and lip color. Other Permanent Cosmetic procedures include stretch mark camouflage, scar camouflage and correcting asymmetries of the face (cleft lip).

Permanent makeup procedures for lip color work beautifully. The color selection can vary from a soft natural pink, for those who want a natural look, to a bold dramatic color. No matter what color you select, this procedure eliminates the need for reapplying lipstick throughout the day.

Permanent Cosmetics for eyebrows can be used to add fullness to the eyebrow by implanting pigment with hairline strokes, eliminating the need to draw on the eyebrow (due to hair loss). Patients can enjoy swimming, sports, and other activities without worrying about their makeup.

If you have difficulty seeing to apply makeup, or you are unable to apply makeup due to dexterity difficulties, or just simply want to eliminate the hassle of applying makeup throughout the day – permanent makeup can save time and stress for those who want to look their best all day.

SmartSkin™ Laser Skin Resurfacing

Utilizing the most advanced laser skin resurfacing technology available to enhance skin tone and texture and diminish the appearance of certain skin conditions. The SmartSkin™ device uses a fractional CO2 laser to reduce the effects of sun damage and the natural process of aging, as well as improve collagen production deep within the skin. Developed by Cynosure, the SmartSkin™ laser offers significant rejuvenation of the face and body without having to undergo a surgical procedure.

The Cynosure SmartSkin CO2 Fractional Laser is our most powerful and effective laser for facial rejuvenation. Treatments result in a few days of redness and peeling, requiring a short period of “downtime,” but the results are evident.  The deeper heating of a CO2 laser also stimulates tightening in the subdermal layers for several weeks post-procedure as an added benefit.

Benefits of SmartSkin™

SmartSkin™ laser treatments are designed to maximize effects while minimizing discomfort. The CO2 fractional laser can address numerous aesthetic concerns efficiently with little to no downtime. Additional benefits typically include:

  • Diminished appearance of wrinkles, age spots, and sun damage
  • Improved skin texture and tone
  • Stimulated collagen production for healthier-looking skin

During your initial consultation, Dr. McMullen will develop a customized treatment plan that can address your unique anatomical needs and expressed goals. Furthermore, he will discuss all the risks and benefits of the procedure and answer any questions you may have.

image_1889829_right The Icon Max G IPL is the best way to treat sun spots and redness on your face and body. An IPL (Intense Pulsed Light) treatment delivers visible results quickly and is a powerful alternative to microdermabrasion and chemical peels. Treatment sessions take about 30 minutes, and there is minimal redness and swelling and no downtime. We recommend 3 treatments scheduled one month apart.

An IPL treatment reduces the appearance of dark spots and pigment on your face and body, including:

  • age spots
  • sun damage
  • solar lentigines
  • moles
  • birthmarks
  • freckles

Patients will notice an improvement in their skin even after the 1st treatment but optimal results will be seen over several months. The results of this procedure are gradual and occur as the skin heals.

image_1889831_right Unwanted hair is a common cosmetic problem for both men and women. Until recently, the only long-lasting method of hair removal was electrolysis. With new laser technology, long-lasting hair reduction can be obtained from any area of the body.

Dr. McMullen and his staff use the Cynosure Elite Laser for fast, painless laser hair reduction. The Elite is unique in having 2 different laser wavelengths, allowing us to treat ANY HAIR COLOR and ANY SKIN COLOR safely and effectively.

Hair reduction treatments are sold in packages of 6 sessions, as it generally requires that many sessions to treat hair in the appropriate phase of its growth cycle to see a significant reduction in hair. No anesthesia is required, but we offer a mild topical numbing cream if the patient so desires.

Upper lips, bikini line, and underarm treatments may take as little as fifteen minutes. Since no procedure can guarantee permanent hair removal, most patients can expect a 75-90% reduction in hair after 6 sessions, leaving any remaining hairs thinner and lighter in color. You should avoid sun tanning eight weeks prior to your treatments.

image_1889833_right Times change, fads change and relationships end. Sadly though, your tattoos won’t get that message. Luckily a tattoo doesn’t have to be forever anymore.

Laser Tattoo Removal uses a very specialized, high power laser to destroy to imbedded intradermal ink and cause fading and disappearance of homemade and professional tattoos. The laser tattoo removal method is proven to remove your tattoo with the least possible scarring and recovery time compared to other methods.  We use the Cynosure Q-Switched Medlite C6 Laser system to deliver the best results for all colors, all skin types, in the minimum number of visits.

Most tattoos require anywhere from 3-12 sessions for complete removal.  Factors for determining how many sessions required include whether the tattoo was homemade or professional, type of ink, multiple colors (green and yellow require the most treatment), and depth of ink.

What makes up a good skin care program?

The five basic steps of a powerful skin rejuvenation program are:

  1. The correct cleansing regime for your particular skin type.
  2. An excellent topical exfoliant to unroof the (dead skin cells) stratum corneum (glycolic, lactic, azeleic acids)
  3. A dermal growth stimulator (tretinoin, retinoids, retinols).
  4. The correct nutrients and antioxidants to promote dermal growth (topical C, topical E).
  5. Photoprotection (Topical C, titanium dioxide) from the sun’s damaging UVA and UVB rays.
What are dermal fillers?

Pharmaceutical fillers are medical compounds or implants, not drugs, that work by replacing lost volume. Soft-tissue fillers are generally considered temporary, semipermanent or partially permanent since the results of treatment will diminish over time.

Your face is always changing as the result of aging and weight fluctuation, so while the notion of a permanent filler may seem attractive, it is often not desirable or appropriate for the long term.

What type of fillers are available?

There are five basic groups of dermal fillers: collagen-based, hyaluronic acid based, synthetic temporary fillers, semipermanent fillers and partially permanent fillers.

What are collagen-based fillers and what are they used for?

Collagen-based fillers are used to fill wrinkles, moderate creases and depressed scars. They are also effective for adding volume to the lips.

An anesthetic may be used to minimize pain. Fillers usually require overfilling or touching up at two weeks to achieve desired results (excess fluid is absorbed by the body over the first few days.) Results last several months.

What is Restylane, and what is it used for?

Restylane is a hyaluronic acid, a natural compound found in human tissue that binds water. It can be used to decrease the appearance of fine lines, creases, depressed scars and is also effective for adding volume to the lips. It is equally effective in decreasing the appearance of frown lines, “smoker’s lines” and prominent nasolabial folds. Restylane is a non-animal hyaluronic acid.

A topical anesthetic or dental block may be used prior to injection.

What is Botox and what is it used for?

Botox is an FDA approved denervating agent is used to treat wrinkles, creases and furrows by temporarily paralyzing the offending muscles, restoring “balance” between the opposing muscle groups.

Botox is botulinum type A neurotoxin that generally requires three to eight pinpoint injections per region. It is used for smoothing out the forehead, crow’s feet, smile lines and other areas where muscle imbalance exists.

Botox treatments have no associated “down time”. Botox requires 5-10 days to achieve its full effect, and that effect can last up to four months.

There is more to healthy skin than just skin care products and treatments. Your lifestyle is a very important, and frequently overlooked, source of skin problems. To maximize the results from your skin care regimen and your treatments at Oklahoma SkinCare, we have developed the following guidelines to help you achieve and maintain beautiful, healthy skin.

I. No smoking or tobacco products of any kind!

II. Dietary Guidelines:

A. Water – Drink a minimum of 8-10 glasses per day. Rule of thumb: 1 oz of water per pound of body weight per day. (120 lb woman needs 10 12-oz glasses per day). Avoid sodas, coffee, caffeine. Decaf hot tea, green tea, iced tea okay.
B. Protein – Most people get too little protein daily. Rule of thumb: 0.5 – 1.0 gram protein per pound of lean body mass per day, just to maintain existing lean mass. (120 lb woman with 20% body fat needs 80 grams of protein per day). 80 grams is about 12 oz of good protein. Eat more fish, chicken, turkey, less red meat.
C. Carbs – Limit potatoes and bread to 2 servings per week. Eat only whole grain bread. Green & yellow vegetable servings are best, keep to serving size = cup (palm of your hand).
D. Fats – Get fats from olives, olive oil, nuts, or avocado with each meal. Serving size = your thumb.
E. Eat 5-6 smaller meals during the day, avoid the “no breakfast, two large meals” regimen.
F. Eat 3 servings of fruit and fresh vegetables per day.
G. Limit snacks, desserts to 2 servings per week.
H. Alcohol – Reduces fat burning up to 42%! One glass Red wine considered “heart-healthy”. Avoid beer, hard liquor!

III. Supplements:
A. Vitamin C – Up to 1000mg daily
B. Vitamin E – 200mg weekly
C. Omega-3 Fish Oils – 1000mg daily D. Multivitamin – once daily (Centrum, Centrum+Iron)

IV. Sleep – 8 hours per night minimum. Additionally, keep your bedtime and wake-up time the same every day (even weekends and vacations!) to keep your cortisol cycle from fluctuating.

V. Exercise  Minimum of 30 minutes, 3 times weekly. Best time is early AM but whatever is convenient so it becomes part of your routine, not a chore. Make time for yourself!

VI. Oral Hygiene – Tooth & gum disease are frequently overlooked health problems which can lead to heart disease and other health problems. Repair and maintain your teeth and gums – it will be one of the most valuable investments you can make in your health.

VII. Regular Check-Ups – Check with your regular physician when making significant changes to your diet or exercise routine, especially if you have specific health problems such as diabetes, kidney or liver problems, hypertension, etc.
